NANCY Grace was re leased from the hospi tal yesterday, five days after being admitted for blood clots in her lungs.

Grace, 48, gave birth to premature twins, Lucy and John, Nov. 4. Last Sunday, she was rushed to the hospital after having trouble breathing.

“She’s home,” Grace’s publicist told The Post yesterday. “She’s not completely out of the woods, but she’s being monitored and she’s definitely on the mend. She has a pretty bad cough.”

Grace, who anchors Headline News’ top-rated “Nancy Grace,” is married to investment banker David Linch.
