With England beating Panama 2-0, Croatia finishes second in the group.
Petar Sucic's moment of brilliance was the difference in a steady first half.
After Nikola Vlasic hit the post, Sucic was found by Mateo Kovacic and fired a daisy-cutter through defensive legs into the bottom-left corner, catching Benjamin Asare unaware just past the half-hour mark.
Ghana switched things up after the break and restored parity in the 73rd minute.
After a VAR check for offside, Derrick Luckassen scored with a smart side-foot from a lovely Ernest Nuamah free-kick delivery.
Despite the celebration, Croatia remained undeterred and quickly restored their advantage.
Mario Pasalic forced a fine save from Asare to win a corner.
From the corner, the Hearts of Oak keeper couldn't react quickly enough to stop Vlasic's pinpoint header down into the bottom-left corner, kissing the inside of the post.
Croatia will now play Portugal in the next round, while Ghana will face Colombia, inadvertently ending Scotland's tournament.
Croatia played above their 0.42 xG, while Ghana also worked well around their 0.74 xG chances.
