A 96-year-old Merchant Marine veteran is facing eviction from his Upper West Side home of over 40 years because he missed deadlines to fill out paperwork, a lawsuit charges.

Harry Woodard’s rent-stabilized apartment at 20 W. 64th St. was deregulated by the state’s Department of Housing and Community Renewal in March after he forgot to submit routine forms to keep the $2,533-a-month two-bedroom, his Manhattan suit says.

Landlord Ogden Cap Properties sent notices to Woodard in 2013 asking him to prove his family’s yearly income was under the limit of $200,000, court papers say.

Woodard lives with his artist daughter. They earn a combined $90,000 a year, according to court records.

Ogden Cap spokesman Fraser Seitel said, “The landlord doesn’t intend to evict him and wants to work with him.”
