An off-duty cop — the star punter for the NYPD football team — was killed yesterday in a horrific Long Island car crash.

Six-year veteran Fred Barraza, 31, was in a 1994 Honda that jumped the curb and hit a tree in Westbury at 2 p.m., authorities said.

Barraza was pronounced dead at Nassau University Medical Center in East Meadow.

Another man in the car, which broke apart on impact, was thrown 50 feet and landed on the sidewalk. He suffered serious head injuries, officials said.

Nassau County Detective Lt. Kevin Smith said cops have not determined who was driving.

Barraza was married with a young son, sources said.
