Butch Patrick — who played Eddie Munster on the ’60s TV show “The Munsters” — checked into a New Jersey rehab center to get help for an addiction to cocaine, marijuana and alcohol, TMZ reported yesterday.

A rep for the 57-year-old actor said Patrick wants to “get a grip on his addiction and educate himself” at the facility in South Jersey.
