Brooklyn hospitals found to be financially struggling would likely be downsized under a state plan unveiled in a report yesterday.

The number of unnecessary beds would be reduced but primary-care services would be expanded.

The Brooklyn Health Systems report, submitted to Gov. Cuomo’s Medicaid Redesign Task Force for review, pronounced six hospitals no longer financially viable.

The panel recommends:

* Merging the Brooklyn Hospital Center, the Interfaith Medical Center and Wyckoff Heights Hospital.

* Coupling the Brookdale Medical Center with the Kingsbrook Jewish Medical Center.

* Accelerating the marriage of the SUNY Downstate Medical Center and Long Island College Hospital by consolidating inpatient services at the LICH campus.

The report also urges Albany to approve a law that would empower the state health commissioner to take over troubled hospitals and replace board members.
