A Queens man committed suicide in front of the emergency room entrance of Elmhurst Hospital early Thursday, police sources said.

The victim, identified by cops as 56-year-old Martin Matulis of Elmhurst, took a cab to the hospital on Broadway near 41st Avenue and shot himself in the head around 5:30 a.m., police said.

Police were called to the scene, where they found Matulis unconscious and unresponsive with a gunshot wound to his head. He was pronounced dead at the scene.

Hospital surveillance footage captured the man shooting himself, sources said.

Authorities recovered a firearm and a suicide note, but it was not immediately clear what the note said, according to police.
