* Police yesterday identified a man who was shot dead outside a Fordham apartment building over the weekend.
Jaci Agosto, 21, was shot several times in the head by two unidentified men at Creston Avenue near East 188th Street about 3:30 p.m. Saturday, cops said.
He was pronounced dead at the scene. The shooters fled.

Sources said witnesses at the crime scene had ID’d Burley to police.
In the early morning of Jan. 6, Burley pulled a gun and shot Corey King, 23, in the head at Asch Loop and Aldrich Street, cops said.

MANHATTAN
* A man was wounded as he fled a party at the Harlem YMCA yesterday – minutes after he shot a reveler while trying to rob him of his jacket, cops and witnesses said.
Ahmad McClure, 21, was shot in the leg at 1:30 a.m. on West 134th Street while running from the party, authorities said.
It is still not clear whether his gun discharged accidentally or if he was hit by a responding officer, Police Commissioner Ray Kelly said.
Minutes earlier, McClure, 16, had shot a partygoer several times, hitting him in the shoulder and face, cops said. Witnesses said McClure had tried to steal a jacket from the teen.
The violence occurred after cops responded to a call to help control the overcrowded party, Kelly said.
As officers called for backup, a fight broke out among a group of partygoers including the teen and McClure, and the teen was shot at, Kelly said.
Kelly said officers encountered McClure running from the party still holding his gun, and either they fired their guns or his own weapon discharged.
McClure was taken to Harlem Hospital, where he is in a stable condition.
The teenager is also in Harlem Hospital, where his injuries are not life-threatening, authorities said.
* Police yesterday released this photo of an 82-year-old man (above) who disappeared from his Chelsea apartment.

The neighbor told Carrero’s ex-wife, who lives in the area. She then notified police at the 10th Precinct on Jan. 12.
Police do not suspect foul play. Anyone with information should call CrimeStoppers at (800) 577-TIPS. All calls will be kept confidential.
BROOKLYN
* A man was shot in the head by an acquaintance as they sat inside a car in East New York yesterday, cops said.
The 29-year-old victim was in the passenger seat of a Lexus ES 300 with North Carolina plates parked outside a barber shop on Williams Avenue at Newport Street just after noon, while another man sat behind the wheel.
A third man, known to both men in the front seat, then slid into the back seat of the car. Not long after, the man in the rear pulled out a gun and shot the passenger once in the back of the head.
The shooter bolted from the car, and the driver sped off with his wounded passenger.
The driver brought the victim to Brookdale Hospital, where he was listed in critical condition.
