A reckless driver T-boned another car while fleeing cops early Thursday in the Bronx, sending four people to the hospital, police said.

An unmarked police car observed the driver of a Mercedes E-350 making an unsafe lane change, and attempted to pull it over around 2:30 a.m. in Eastchester, cops said.

The driver then sped off down the wrong lane and slammed into a Chrysler Town and Country just a few blocks away, police said.

Two men and one woman in the Chrysler, as well as a passenger in the Mercedes, were taken to Jacobi hospital with minor injuries.

Police have the driver in custody, and charges against him are pending.
