Four car accidents cost five people their lives during a 10-hour stretch in the city over the weekend.

Dawn Affoumani, 42, a seventh-grade schoolteacher and mother of two, was killed just blocks from her Bronx home when she was struck by a tan Mazda at 9:45 p.m. Saturday while crossing White Plains Road at Story Avenue. The Mazda’s driver fled the scene, cops said.

Just before 3 a.m. yesterday, a 26-year-old motorist heading north in a southbound FDR Drive lane slammed his Nissan Maxima head on into a Dodge Caravan near the Houston Street exit, authorities said.

Both he and car-service driver Miron Shlepakov, 62, of Brooklyn, died at the scene. In addition, a 22-year-old woman’s Mazda struck the Dodge Caravan, injuring her and her 31-year-old passenger. Both were in stable condition last night at Bellevue Hospital.

The fourth victim was Darren Green, 41, of Jamaica, Queens. He lost control of his livery cab, a Lincoln Town Car, just after 4 a.m., struck a utility pole and was fatally flung onto South Conduit Avenue near Linden Boulevard in Ozone Park.

At 7:41 a.m., a hitandrun driver killed Luis Rosado, a 75-year-old great grandfather from Harlem, as he crossed Broadway at West 138th Street, cops said.

Rosado, a retired baker who had braved the icy Sunday morning temps to attend Mass, was pronounced dead at the scene.
