An Upper East Side woman leaped to her death from the roof of her swanky apartment building yesterday, authorities said.

Police sources identified the Manhattan woman as Emylia Slivka, 37, and said she had a history of mental illness.

Slivka jumped from 240 E. 86th St. at about 9 a.m., a witness said.
