A Bronx warehouse worker died early Thursday after his head was crushed while he operated a forklift, police said.

Shakeem Farnum, 43, was driving the forklift at Chef’s Warehouse Food Distribution Center in Hunts Point at about 1 a.m. when his head became wedged between shelving and the machine, according to cops.

Farnum, who police said suffered extreme head trauma, was pronounced dead at Lincoln Hospital.

“We are heartbroken to learn of the tragic accident that took the life of our friend and colleague,” Chef’s Warehouse said in a statement, adding it was committed to safety and would aid any investigation.

The Occupational Safety and Health Administration was investigating. It said it had received no prior complaints against the food distributor.
